Understanding the Medication Sacituzumab Govitecan 認識化療藥品Sacituzumab govitecan

Drug Introduction
Sacituzumab govitecan is an antibody–drug conjugate (ADC) that combines a monoclonal antibody with a small-molecule cytotoxic drug, specifically targeting and killing tumor cells.
Currently, the Taiwan Ministry of Health and Welfare has approved its indications for the treatment of:

  1. Locally advanced or metastatic triple-negative breast cancer (TNBC)
  2. Locally advanced or metastatic hormone receptor–positive, human epidermal growth factor receptor 2 (HER2)–negative breast cancer

How Is It Administered?
The first infusion should be given intravenously over at least 3 hours. The duration, intervals, and number of subsequent infusions are determined by the physician’s clinical judgment.

Precautions Before and During Administration

  • Before starting treatment, inform your doctor if you have any of the following conditions:
    • Liver disease
    • Kidney disease
    • Current use of certain medications, including anticonvulsants (Carbamazepine, Phenytoin), antibiotics (Ketoconazole, Rifampin), and anti-HIV agents (Ritonavir, Tipranavir)
  • Sacituzumab govitecan may lower white blood cell counts, increasing the risk of infection. During treatment, avoid crowded places, maintain good personal hygiene, and seek medical attention promptly if you develop fever, chills, sore throat, or flu-like symptoms.
  • It may also lower red blood cell counts. Notify your doctor immediately if you experience unusual fatigue, dizziness, chest tightness, or palpitations.
  • Pregnancy should be avoided during treatment. If pregnancy is suspected, inform your doctor immediately.
  • Sacituzumab govitecan may affect fetal development. Women should use effective contraception during treatment and for at least 6 months after the last dose; men should use effective contraception for at least 3 months after the last dose.
  • Breastfeeding should be avoided during treatment and for at least 1 month after the last dose.
  • Sacituzumab govitecan may cause severe diarrhea or severe nausea and vomiting. Notify your doctor promptly if diarrhea occurs for the first time. If diarrhea lasts more than 1 day, if nausea/vomiting prevents fluid intake, or if symptoms of dehydration occur (such as dizziness, decreased urine output, rapid breathing, or fast heartbeat), seek medical attention immediately.

Possible Side Effects

  • Allergic reactions: During infusion, if you develop itching, rash, swelling of the face or hands, swelling or tingling of the lips or throat, chest tightness, difficulty breathing, dizziness, or palpitations, notify healthcare personnel immediately.
  • Nausea and vomiting: Your doctor will prescribe antiemetic medications to prevent nausea and vomiting. If symptoms do not improve despite taking antiemetics, inform your doctor.
  • Diarrhea: Adequate fluid and electrolyte replacement is recommended to reduce the risk of dehydration and kidney strain. Once symptoms improve, resume eating gradually starting with liquids or semi-liquids, progressing to soft foods.
  • Bone marrow suppression: Includes reduced white blood cells, platelets, and red blood cells, which may cause fatigue, decreased immunity, and increased bleeding risk. During treatment, avoid infections, monitor for abnormal bruising or pinpoint bleeding, and report any abnormalities to your doctor immediately.
  • Other common side effects: Rash, itching, hair loss, fatigue, loss of appetite, constipation, and abnormal liver function tests.

Dietary Considerations

  • Inform your doctor if you are taking any over-the-counter medications, herbal remedies, or vitamin supplements.
  • Avoid raw or undercooked foods, as well as expired or spoiled food, during treatment to reduce the risk of infection due to lowered immunity.
  • If you have special dietary needs or questions, you may schedule a nutrition consultation for personalized advice from a dietitian.
