乳癌質子治療

質子治療乳癌癌症治療精準醫療放射線治療質子

乳房放射治療是運用高能量放射線，照射乳房與鄰近淋巴區域，多數用於手術後之輔助治療，以消除手術後肉眼及影像難以偵測的殘餘癌細胞，降低癌症復發風險；無法手術的個案亦可由團隊評估放療之可行性。

乳房位於心臟與肺臟前方，傳統光子治療(Photon Therapy)在穿透乳房後，仍會有殘餘劑量進一步影響心臟與肺臟(圖一.二)，可能增加重大心血管疾病、肺部發炎或其他次發性癌症風險。

質子治療(Proton Therapy)是運用質子束，能精準將高劑量能量集中於腫瘤位置，降低後方正常器官所受之劑量及影響。根據醫學實證，與傳統光子治療相比，乳癌質子治療可減少約75–90%的心臟輻射劑量，並降低約50–75%的肺部輻射劑量，大幅減少對重要器官的潛在傷害(圖三)，有助於降低副作用，提升治療後的生活品質。

加速部分乳房治療(Accelerated partial breast irradiation, APBI)是早期乳癌患者於乳房保留手術後放射線治療選擇之一，與過往需照射整個乳房不同，此方式僅針對「腫瘤原發位置及其周邊高風險區域」進行照射，結合質子技術運用於符合適應症患者(圖四)，可達到與全乳照射同等治療效果，亦降低正常乳房組織及重要器官的照射劑量，有助於維持乳房外觀，減少乳房縮小、皮膚發炎與疲倦感等副作用，還能大幅縮短療程，減少往返醫院次數。

質子治療在兼顧治療效果與安全性的同時，更能保護重要器官，是乳癌患者追求生活品質與長期健康時，值得考慮的放射治療選擇。
