Guideline for Outpatient Service

1. First time visit
(1)Methods of Registration Appointment
■On-site registration
■Manual phone registration
(2)Please go to the Information Counter to fill out the first-time appointment form.
(3)Please check in at the registration counter with your ID card/Residence Certificate/Valid passport, National Health Insurance IC card and the first-time appointment form.
(4)Please perform physical examination including weight, height and blood pressure at the clinic waiting area in each of the medical buildings.
(5)Please report to the consultation room with an empty medical chart folder issued by hospital.

2. Return visit
(1)Methods of Registration
■On-site registration
■Manual Phone registration
■Schedule an appointment by your doctor during consultation
(2)Blood pressure measurement（if necessary）

3. All patients please follow the subsequent procedures as below：
(1)Wait for your queue number to be flashed on the display before proceeding to the consultation room
(2)Physicians’ consultation
(3)Proceed medical laboratory test or examination ordered by the physicians and then bring test results to the consultation room if necessary（on the same day or the other day）
(4)Pay your fee at the cashier/registration counter
(5)Go to the admission center for hospitalization or the pharmacy for medications if necessary
(6)Go to the injection room if necessary
(7)Leave the hospital
